The following is to be carefully read and signed by prospective volunteers/participants. Should prospective volunteers/participants be less than 18 years of age (“minors”), the following is to be executed by the prospect’s parent/legal guardian. Enrollment of the volunteer/participant named below shall not be accepted by Project LifeForce (herein named “PLF”) unless this Consent Waiver and any addenda is signed.

 

Liability Clauses

1. I am aware that participation in any activity can be dangerous, involving many risks including serious injury or death. I hereby assume all the risks associated with my participation. I understand that PLF does not provide medical and/or accidental insurance for camp volunteers/participants.

2. Knowing and having been informed of the potential dangers and risks associated with programs and in consideration of participation in camp, I hereby agree on behalf of myself, my family members to assume all such risks. I further agree to waive, release, discharge and hold harmless any camp chosen facility, the Camp Director, PLF programs and their respective employees, agents, representatives, first responders, students, and other affiliates from any and all liability, actions, causes of actions, claims or demands for personal injury, death or property damage of any kind or nature, and any other claims whatsoever arising out of, or in any way connected with, my participation in PLF programs and/or activities.

3. PLF and their respective employees, agents, representatives, first responders, students, and other affiliates shall not be responsible for clothing, jewelry, cell phones, or other personal possessions that are lost, stolen or damaged.

• This Consent Waiver and its addenda extend to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown. The terms hereof shall serve as an assumption of risk, release and waiver for myself, my family, my child/ward and our heirs, executors, administrators, guardians or anyone else who might assert a claim on our behalf.

4. I hereby consent to permit all staff working at the camp to provide emergency first-aid or medical treatment, according to their best judgment, in the event I suffer an injury or illness while participating in the camp or on the camp premises. I also understand that my primary health insurance is to be the only insurance responsible for any accidents or potential claims that occur at camp and that PLF and their respective employees, agents, representatives, first responders, students, and other affiliates are in no way responsible for any injury or for payment via insurance for any injury or other issues I may come across.

 

Photography Clause

5. I hereby grant PLF and/or its designated representatives the right to take, edit, alter, copy, exhibit, publish, or distribute photos, videos, and/or audio of my participation in camp. I further authorize the use and reuse of said products for any and all educational and public services, advertising or marketing efforts, and relinquish any and all rights, title and interest I may have in said photographs, movies, videos, finished pictures, audio, reproductions, copies or negatives of the same in connection with such uses.

 

Behavior/Conduct Clause

6. PLF reserves the right, at the sole discretion of the Camp Director, to dismiss any volunteer/participant whose conduct or influence is unruly, disruptive, uncooperative, or in the opinion of the Camp Director, contrary to the best interest of the PLF program and its Mission*. Among these offenses are unauthorized departure from PLF grounds or any violations of any of the conditions outlined in this Consent Waiver and its addenda. In all matters of violation, those outlined in this Consent Waiver and its addenda and those not, there will be no refund of any part of the program fee.

 

7. PLF may cancel camp sessions due to low enrollment. We strive to provide volunteers/participants notice of cancellation at least 7 days prior to the beginning of the camp session. Refunds for payments made by credit card will generally be refunded to the same credit card used to pay for enrollment. Refunds for payments made by check or cash will be refunded by check. All refunds due to camp session cancellation will be made within 30 days of the notification to withdrawal.

 

CONSENT WAIVER ADDENDUM

 

Photography Clause

1. Unless expressly authorized by the Camp Director, I understand that under no circumstance may I photograph, video, or audio record camp PLF’s respective employees, agents, representatives, first responders, volunteer/participants, students, or other affiliates. I further understand that any such recordings made in participation with PLF or on PLF grounds is considered the property of PLF. I relinquish all rights, title, and interest I may have in said photographs, movies, videos, finished pictures, audio, reproductions, copies or negatives of the same. I understand being in unauthorized possession of said property may result in expulsion from the program.

 

Behavior/Conduct Clause

2. I understand that my conduct should align with the Mission* of PLF. In comprehending this, I further understand that I may not act in a manner that discriminates against any other PLF employee, agent, representative, first responder, volunteer/participant, student, or other affiliate on the basis, including, but not limited to, age, race, sex, gender, religion, marital status, or sexual orientation. Any conduct identified that violates this clause may result in expulsion from the program. 

 

PLF retains the right to edit, modify, supplement, or amend this Consent Waiver and any addenda at any time without notice to signors.
